What kinds of insurance do you accept?

What are your hours?

Clinic hours 7:00 AM to 6:00 PM Monday through Friday

Administrative hours 8:00 AM to 6:00 PM Monday through Friday

Do you treat Workers Comp cases?

Yes.  Please contact our office to learn more.

How long is a treatment visit?

On average, an evaluation will take approximately an hour and a half, and a returning visit will approximately be 45 minutes to an hour.

How early do I need to show up prior to my first appointment time?

We request that all patients arrive 30 minutes prior to their appointment time to complete paperwork unless paperwork has been printed from the website and completed at home.  If the paperwork has been completed at home we request that you arrive 10 minutes prior to your appointment time.

Do I need a referral from a physician prior to making an appointment with a physical therapist?

Yes, a referral from a physician is required.

What type of clothing should be worn to a physical therapy visit?

Comfortable clothing is recommended.  It will depend on what body part is being treated.

  • lower extremity – shorts
  • neck and shoulder – tank tops
  • back – nothing too tight around the waist

Gowns are also available for your convenience.

Do I need to stay with my minor during their appointment?

Yes, we require a parent be present during their childs appointment.

Is child care available?

No, we do not provide childcare.

What administrative information do I need on my first visit?

You will need to bring your insurance card or billing information.  We also need a referral from your physician and a copy of your drivers license.

If I am a Medicare patient receiving any home health services can I be treated in your office at the same time?

No, Medicare requires all home health services to be completed prior to outpatient treatment.  Once you have been completely discharged from the home health agency we will be able to schedule an appointment.
